An email was sent to the address you provided your dentist with instructions to register your username for the Responsible Portal. Be sure to check your spam filter if you do not receive the registration email.

Select "Log In" from the menu of this page and select "Create a UserName" then enter the email address you provided your dentist. Follow the instructions on the email to set up your username. Be sure to check your spam filter if you do not receive the email.

Select "Log In" from the menu of this page. Above the "Password" input field, click on the link named "Forgot". You will be prompted to enter your email address and an email will be sent to you with instructions on resetting your password.

For questions about your treatment, please contact your Dentist’s office.
